Dental, vision and hearing with Medicare: what's covered, and the ways to add it

Original Medicare doesn't cover most dental care, eye exams for glasses or hearing aids. Let me show you what it does cover, what a Medicare Advantage plan may add, and where other coverage fits.

In most cases, Original Medicare doesn't cover dental services like cleanings, fillings and dentures, eye exams for prescription glasses, or hearing aids and the exams to fit them, so you pay those costs yourself unless other coverage helps. Some Medicare Advantage Plans offer extra benefits for vision, hearing and dental; check with the plan. Medigap policies generally don't cover them.

Dental

Original Medicare
In most cases it doesn't cover routine cleanings, fillings, tooth extractions, dentures or implants. It may cover some dental services tied to certain covered treatments, like an oral exam before a heart valve replacement or an organ transplant, or some services when you're admitted to a hospital for a dental procedure because of a medical condition or the procedure's severity.
A Medicare Advantage Plan may add
Some plans offer dental services as an extra benefit. Contact the plan for what it covers.
Medigap and other coverage
Medigap policies generally don't cover dental care. Once you have Medicare, in most cases you can't buy a stand-alone dental plan through the Health Insurance Marketplace.

Vision

Original Medicare
It doesn't cover eye exams for prescription eyeglasses, and Part B doesn't usually cover eyeglasses or contact lenses. Part B does cover one pair of eyeglasses with standard frames, or one set of contact lenses, after each cataract surgery that implants an intraocular lens.
A Medicare Advantage Plan may add
Some plans offer vision services as an extra benefit. Contact the plan for what it covers.
Medigap and other coverage
Medigap policies generally don't cover vision care or glasses.

Hearing

Original Medicare
It doesn't cover hearing aids or exams for fitting them. You pay all costs.
A Medicare Advantage Plan may add
Some plans offer hearing services as an extra benefit. Contact the plan for what it covers.
Medigap and other coverage
Medigap policies generally don't cover hearing aids.

When Original Medicare does pay, and your share

There are a few more cases where Original Medicare helps with dental or vision costs, and you still pay a share when it does. For hearing aids, the answer is short.

Dental

  • A dental service may be covered when it's linked to the success of a covered medical treatment. That can include a procedure like a tooth extraction to treat a mouth infection before chemotherapy, and treatment for a complication during head and neck cancer treatment.
  • If you have End-Stage Renal Disease (ESRD), it may cover dental or oral exams, and medically necessary tests and treatment to remove an oral or dental infection, before and while you get Medicare-covered dialysis.
  • For a dental service Part B covers, you pay 20% of the Medicare-approved amount after you meet the Part B deductible, plus a copayment to the facility if you get it in an outpatient hospital or other facility setting.

Vision

  • For the glasses or contact lenses Part B covers after each cataract surgery with an intraocular lens, you pay 20% of the Medicare-approved amount after you meet the Part B deductible.
  • You pay any additional costs for upgraded frames.
  • Medicare only pays for glasses or contact lenses from a supplier that participates in Medicare, whether you or your provider submits the claim.

Hearing

  • Original Medicare doesn't cover hearing aids or the exams for fitting them, and you pay all costs. Unlike glasses after cataract surgery, Medicare.gov names no case where Original Medicare pays for them.

Besides a Medicare Advantage Plan, a Medicare Cost Plan or a Program of All-inclusive Care for the Elderly (PACE) plan may cover some extra benefits Original Medicare doesn't, like certain vision, hearing and dental services.

Extra benefits inside a Medicare Advantage Plan

Medicare Advantage Plans must cover all medically necessary services Original Medicare covers, and plans may also offer extra benefits Original Medicare doesn't cover, like some vision, hearing and dental services. You need both Part A and Part B to join, you keep paying your Part B premium, and you may need to use the plan's network.

What decides what you pay

For services Original Medicare doesn't cover, including most dental services, you pay all costs. For a covered service, what you owe depends on:

  • Other insurance you may have
  • How much your doctor charges
  • Whether your doctor accepts assignment
  • The type of facility
  • Where you get your test, item or service

Ask your doctor how much a test, item or service will cost, why it is recommended, and what Medicare will cover.

What to check with a plan before you count on these benefits

Extra dental, vision and hearing benefits differ from plan to plan, so check with the plan before you enroll: what benefits it offers, whether you might qualify and whether there are any limitations. These are the questions worth asking.

  1. Which providers can I use?

In a Medicare Advantage Plan, you may need to use providers in the plan's network and service area for non-emergency care. Some plans cover care out of network, but it may cost more.

  2. Do I need a referral or approval first?

You may need a referral to see a specialist, and you'll typically need prior authorization from the plan before it covers certain services or supplies. If the plan says a service isn't medically necessary, you may have to pay all its costs, but you have the right to appeal.

  3. What will I pay?

Many plans charge a premium in addition to the monthly Part B premium, and out-of-pocket costs vary by plan and can differ for certain services.

  4. Where are the details written down?

Each fall your plan sends an Evidence of Coverage, with details about what the plan covers and how much you pay, and an Annual Notice of Change, with changes that take effect in January. If you don't get them, contact your plan.

  5. Will the plan be here next year?

Insurance companies decide where a plan is offered and can decide each year to join or leave Medicare. If your plan stops participating, you'll have to join another Medicare health plan or return to Original Medicare.

Every kind of coverage, explained

Dental, vision and hearing: questions you might ask me

Is dental, vision or hearing insurance worth it?

It depends on the care you expect and on what a policy or plan would pay for it. Original Medicare doesn't cover most dental care, eye exams for glasses or hearing aids, so without other coverage you pay those costs yourself. A policy's premium, what it covers and its limits are in the insurer's quote and the policy's own terms, and you check a Medicare Advantage Plan's extra benefits with the plan before you enroll. I'll give you my own answer when you first get in touch. (Source: Medicare.gov, What's not covered and Your health plan options, 2026.)

Can I buy dental insurance on my own if I have Medicare?

It depends on where you buy it. Original Medicare doesn't cover most dental care, and Medigap policies generally don't either. Through the Health Insurance Marketplace, in most cases you can't buy a stand-alone dental plan once you have Medicare. Some Medicare Advantage Plans include dental services as an extra benefit. Whether an insurance company will sell you a dental policy outside those, and on what terms, comes down to the insurer's quote and the policy itself. Ask me about your own situation when you first reach me. (Sources: Medicare.gov, Dental services and Learn what Medigap covers, 2026; HealthCare.gov, Medicare and the Marketplace, 2026.)

What does dental insurance cost for someone on Medicare?

It depends on the policy or plan. A dental policy's premium is in the insurance company's quote and the policy's own terms. In a Medicare Advantage Plan, premiums and other costs vary by plan and can change each year, and you keep paying your Part B premium. With Original Medicare alone, you pay all costs for most dental services. I'll go over your own situation when you first get in touch.
